  1. Download application form ( MS Word Format or Adobe PDF Format ), fill out (English ONLY) and signe.
  2. Download Credit card charge authorization form (MS Word Format or Adobe PDF Format ).
  3. Self-addressed stamped envelope for return mail or prepaid Express Mail envelope, in case, the order is urgent
  4. Fax the above documents to: (212) 233-0916
  5. On the basis of the Application Form we will prepare the Invitation Letter and send it to you by mail.
