FAQ / How do I change my name on my passport?


If your name has been legally changed because of marriage or a court order within one year of your passport being issued, submit a completed U.S. Passport Re-Application Form, (DS-5504), a certified copy of your marriage license or your name change court decree, two recent photos and your current passport. There is no charge unless it has been more than a year.
